AI agent creates duplicate flows

Agent sometimes creates new, duplicate flows when asked to check/verify correct functionality of an existing flow(Decision Orchestrator and FetchState flow in this example).

Seems to happen when user tries to verify functionality/repeat a request regarding a certain flow.
